Desktop Installation Request Form

Use this form to request the installation of a TSO-managed operating system on a newly purchased desktop system or for the re-installation of an older desktop. If this is a new desktop, you will first need to request a DNS name for the system using the DNS Request Form. After you receive a DNS name for your desktop, you can use this form to request the operating system installation. TSO provides managed loads of Windows, Red Hat Enterprise Linux, Ubuntu and Mac OS X operating systems. TSO-managed desktops reside on Inland networks only. Please do not use this form for Outland installation requests (those installations are the responsibility of the end-user).
