The issue was first noticed at 11:20 AM and was reported resolved as of 2:00 PM on Monday, February 1, 2016.
WHY DID IT HAPPEN?
The cause was not communicated.
WHO WAS AFFECTED?
Any user who has enabled dual authentication for their GT account.
WHAT DO YOU NEED TO DO?
No user action is required.
HAS THE ISSUE BEEN RESOLVED?
Yes. OIT patched the service and DUO is once again operating normally.
