WHY IS IT HAPPENING?
Some users have experienced login issues when attempting to access the CoC staff feedback form.  TSO identified an issue on the hosting server edam.cc.gatech.edu that is causing LDAP Bind issues.  TSO will move the application to a new server to reduce user impact while we investigate the root cause.


WHO IS AFFECTED?
Users attempting to fill out the staff feedback form.


WHAT DO YOU NEED TO DO?
No user action is required.
